Final Paperwork reminder!!

All paperwork and Fee’s must be received by the Wolverines before your son/daughter can take the field on the first night of practice. Cheerleaders begin practice on Monday, August 2nd Football begin practice on Monday, August 16th The following is a list of forms/documents that we must have on file: • Signed Registration Form • Signed Consent and Release Form • Signed Minor Photo Release Form • Signed Waiver & Release of Liability Form • Signed Parent / Athlete Participation Agreement • Signed Medical Authorization Permission Form • 2010 Parent / Athlete Expectations Form • League Insurance Waiver (NJHFL Waiver) • Physical Exam Report DATED 2010** (signed by a Medical Professional) • Copy of Birth Certificate • Final Report Card for 2009 – 2010 School year All Wolverines or League forms can be downloaded from the Wolverine web site. They are located either in the Registration Package or listed individually in the Schedule/Forms section. NOTE: To avoid late mail delivery of outstanding forms, if possible, please scan/email your child’s outstanding documents to bwright@alicaremed.com Receipt of forms in this manner allows for immediate printing/filing/completing of the registration packet and ensures your child’s participation on the first night of practice.
